Both parties need to be very self aware and cognizant of the others feelings. And should be constantly checking to make sure things are ok. I don't know if you have already. But you should speak to your partner about what happened. Let him know how you felt. Also in the future you should both strive to have clearer forms of communication. As for placing blame. That's tough. Should he have noticed? Maybe he could have. I don't know what was going on in his head. Totally possible that he could have been so into what was going on that he was oblivious to everything else. You'll have to ask him about that. And I'm not saying that your feelings are not valid. But just be aware that many people are not good at reading body language or good at non verbal communication. Lastly, If you aren't already speaking with a therapist. Please do so.

2. There’s ZERO! need to bring up your ex in order to communicate what type of sex you want. Ask her to go to a sex therapist/coach with you if you’re unable to communicate it yourself.
