Was it like this from the beginning of the relationship? Or has this started during and only gotten worse? I’d probably suggest doing what I already said with a note or text and see how it goes. Just make sure you’re not coming off judgy or angry. Try to have some compassion or grace for her because she sounds to be struggling with it from what you’ve said. If she reacts poorly to that, I think your only option is to say that you care for her and want the relationship to continue, but you can’t continue it with this elephant in the room (her being blatantly dishonest with you). You can even say that you’ve probably messed up too (if you think you have) and that you would work on those issues as well. But at the end of the day, if you’re not happy living like this you have to leave. You can’t make somebody do something they don’t want to do. If she can see that her behavior is hurting your relationship, hopefully, she’ll want to fix it. But if she won’t, then you’re kind of out of options.
